UNDER THE LAW ON THE PROTECTION OF PERSONAL DATA
 DATA SUBJECT APPLICATION FORM
 

Regarding your rights and requests set forth in art. 11 of the Law on the Protection of Personal Data No. 6698 (“KVKK”), you may apply to the data controller, Gungor Law Office;
 

1.  For learning whether your personal data is processed,

2.  If your personal data has been processed, requesting information about it,

3.  To learn the purpose of processing your personal data and whether used in accordance with the purpose,

4.  Knowing the third parties to whom your personal data is transferred domestic or abroad,

5. Requesting correction of your personal data if it is incomplete or incorrectly processed,

6. Requesting the deletion or destruction of your personal data in case the reasons for processing disappear, in order to be evaluated within the principles of purpose, duration and legitimacy,

7. In case of correction, deletion or destruction of your personal data; requesting the notification of these transactions to the third parties to whom the personal data has been transferred,

8. Object to result; if the result arises against you, if your processed personal data is analyzed exclusively through automated systems,

9. In the event that your personal data is processed unlawfully and you suffer damage for this reason, you have the right to demand the compensation of the damage.
    

Pursuant to art. 13 of the KVKK and art. 5 of the Communiqué on Application Procedures and Principles to Data Controllers, it is necessary and sufficient to submit your requests as data subject to Gungor Law Office by choosing one of the following methods and addresses;
 

Consider while applying:

• In the application; the name, surname and if the application is written; the signature, for the Citizens of the Republic of Turkey T.C. Identity Number, nationality for foreigners, passport number and identification number, if any, place of residence or workplace address for notification, e-mail address for notification, if any, telephone number and subject of request must be specified. Information and documents related to the subject are also attached to the application.

• In the application containing your explanations regarding the right that you have as the "Data Subject" whose personal data is processed and that you will make and request to exercise the above-mentioned rights; the subject you request must be clear and understandable, the subject you request is related to yourself or if you are acting on behalf of someone else, you must be authorized and document your authority, the application must contain your identity and address information, and documents confirming your identity must be attached to the application.

• In order for a person other than the data subject to make a request, there must be a power of attorney issued by the data subject on behalf of the person to apply.

• Your applications within this scope will be finalized as soon as possible and within 30 days at the latest. These applications are free of charge. However; if the transaction requires an additional cost, the fee in the tariff determined by the Personal Data Protection Board may be charged.

• In order to ensure the security of your personal data, after your application for information is received, Güngör Law Firm will be able to contact you to confirm that you are the data subject, and may request some documents and information from you in this regard.
